The video is clear and shows that the individual in question did RDM.

However, you don't have his STEAMID, tbh that is actually the most important part of making a warn request.

This is a mistake made by me. 

Your ban should have been shortened to two days, no worries.

Also, an addition;

Your PC didn't crash. The logs was showing it clearly, "disconnect by user".

So, you have been banned for that. If that is not manuel and on purpose, there is no reason to get unbanned.

This is literally Breaking FearRP. In fact, Barring was a bit lenient, you should have also be warned for "Countering Superior Forces" as well.

Apart from being new on the server, I think the answers to the respective questions are poorly made. You didn't answer many questions, and Ustrafe is right. I'm questioning if this is a troll application or not. 
However, you should keep in mind that you should always try to go for the best. What I mean is, if you are making an application, try to do your best. It doesn't matter if the application gets denied or not, the point is to prove yourself and for the future, maybe, re-applying.

Next time, you should follow the template while making a ban/unban request on forums. 
Otherwise, your request is most likely wouldn't be reviewed.

Has no life, mature and competent enough to be a staff member. He is active as well as far as I am aware.

I have noticed that you are quite active and you can make your men listen to what you say which is essential.

Application is insufficient and there are a lot of mistakes, however, in the future, you can learn from your mistakes and maybe improve yourself.

One more thing, as Wiegland said, you replied to your application.

Trustworthy member of the community, stable. The application is good enough for a normal one as well.
He has no microphone but I don't think that makes him unable to perform all staff duties.
