Have you read the SCP-096 rules and understand them? Demonstrate your understanding with an example or two: Yes I read them and understand them.

Are you able to play the job often and RP correctly?: Yes I will try to play as 096 as often as I can and RP correctly.

What is the containment procedure for SCP-096 if it has breached? Explain with as much detail as you can: After 096 terminated it's target 096 will sit calmy on the ground. Foundation personnel is to look away until a containment unit arrives and backs up to 096 will avoiding looking at 096 until the unit feels 096 and bags the entire face so that no one looks at the face, after that they use the containment cuffs to cuff 096 and escort him back to it's CC, no foundation personnel should be in the way or try to remove the bag off 096's face. after 096 is back in it's CC, it will be uncuffed and the unit leaves without looking at 096's face.

While in a test, the D-Class they have put in your Containment Chamber has run out and managed to get to the Entrance Zone after looking at your face. What course of action do you take and what do you do once your target is dead?: I will chase the D class and terminate any person that is blocking f.e. the doors until I kill the D class and sit down where I killed him until someone either looks at my face or I get bagged.

While chasing SCP-096-1, you pass a researcher, MTF personnel and D-Class who all look at your face accidentally. You manage to kill your original target and you circle back to kill the rest. Is this a good RP scenario and why? (Free form question.): The 3 people that looked at my face become 096- II and I have to kill them because they looked at my face, I cannot kill people that didn't look at my face, only my new Targets.

A Group of Interest has entered the facility and is attempting to bring you to their base. While transporting you, one of the members sees your face, however says that it was an accident and that you should just ignore it. What should you do and why?: I would ignore it the first time, if it happens again I will chase them and kill them since the first time it was an Accident and would ruin the RP if I killed him and could lead to others looking at my face by accident

I know your name but that really is it. I barely ever see you and whenever i do, its just from passing by thus i cant tell anything about you.
Your playtime is fairly low but still above the mark so thats fine.

Your third answer is quite invalid as people who trigger your swep, must be killed. It would indeed "ruin rp" if youd decide to ignore as this is one of the most plain and basic rules youd be breaking about 096.

This is a main concern, I am accepting you due to the rest of the application being good, however you must always kill the target, even if a GOI says it was a mistake.
